Payment is expected the date of service and may be paid using any of the following:
Cash, Zelle, or Charge using Visa, Mastercard. FSA / HSA cards accepted.

Out-of-Network Insurance Benefits may be available
We understand that navigating insurance can be confusing. Many patients are surprised to learn that their insurance plan may include Out-of-Network benefits to help offset the cost of physical therapy, while other while other plans may not offer any reimbursement. We recommend contacting your own insurance by calling the member services number on the back of your insurance card to learn about benefits offered by your plan.

Specifically, we suggest inquiring the following:

Whether your plan includes Out-of-Network coverage

Whether you have an Out-of-Network Deductible and how it applies for physical therapy

What percentage of services may be reimbursed

Inquire if you need a referral to begin physical therapy (this is your responsibility)

What is the process to submit claims to my insurance company

After scheduling an appointment and seeing your physical therapist, documentation for evaluations and treatment visits and progress notes are completed to comply with all legal licensure requirements. Upon completion of notes, a superbill (detailed receipt) will be generated that can be emailed to you directly or provided to you at a subsequent appointment. Superbills contain all of the necessary information required for you to self-submit to your insurance. ​

If you have a Health Savings Account (HSA) or Flexible Spending Account (FSA), you may use those funds for our services. ​​

Motor Vehicle Accidents:
We are happy to provide care for patients involved in motor vehicle accidents, however, patients would pay cash for PT services provided and then self-submit to your personal insurance company.
Workman’s Compensation:
We are unable to accept Workman's Comp patients at this time.

Can we see MEDICARE PATIENTS?

We are presently NOT enrolled with Medicare and
cannot submit claims to Medicare at this time.
